Help with the FORM app: how to reach us, delete your account, report or block someone, and manage a FORM Pro subscription.

If you are writing about a problem, it helps to include your iPhone model, your iOS version, and what you tapped before it happened.

Delete your account

You can delete your account yourself, inside the app:

Tap the profile icon at the top of the Dashboard, then Settings.
Scroll to Account and tap Delete Account.
Confirm on both prompts. Your account and its data — including your workouts, programmes, progress, messages, meal photos and exercise videos — are permanently deleted. This cannot be undone.

Deleting your account does not cancel a FORM Pro subscription. Cancel it first (see below), or Apple will keep renewing it.

When the account is deleted, FORM also tries to remove the workouts it saved to Apple Health on that iPhone. You can delete any that remain in the Health app.

Want a copy of your data first? In Settings → Account, tap Export my data.

If you can no longer sign in, email us from the address on your account and we will delete it for you, as described in our Privacy Policy.

Report abuse or block someone

Report a message: in a conversation, long-press the message and tap Report.
Block someone: in a conversation, tap the options button (⋮) at the top and choose Block. A block stops messages in both directions straight away.
Report a trainer’s profile: in the trainer list (when the trainer list is available), tap the flag icon on their card and describe the issue.

Reports are queued for review and acted on within 24 hours. You can also email us at the address above. The Community Guidelines, which you can open in any conversation in the app, explain what is not allowed.

FORM Pro subscriptions

FORM Pro is sold through the App Store and billed by Apple to your Apple ID. FORM cannot cancel or refund an App Store purchase for you.

Cancel or change a subscription: open the iPhone Settings app, tap your name, then Subscriptions, and choose the Form subscription. If you have FORM Pro you can also tap the profile icon in FORM, then Premium (“Manage your subscription”), which opens the same Apple page.
When a cancellation takes effect: at the end of the current billing period. You keep FORM Pro until then.
Restore a purchase: if FORM Pro does not show on a new phone, tap the profile icon, then Restore Purchases.
Refunds are handled by Apple at reportaproblem.apple.com.

Apple Health and AI features
